   How you can begin

Individuals come to the Human Performance Center in a variety of ways. You or a family member can contact us and set up an appointment. Or your physician or other health care provider can refer you. In either case, we coordinate our care with your personal physician or other care provider. In order to bill your insurance company, we’ll need a medical diagnosis. Care at the Human Performance Center involves:

 

1. A thorough examination.

2. Formulation of a plan of care.

3. Interventions - therapeutic or preventive.

4. Individual instruction & education

5. Home Program/Community Exercise.

 

We urge you to become an active partner in your care as opposed to a passive recipient. This may include performing home exercises or, improving your posture, using appropriate lifting mechanics, or modifying your work environment.

The number of visits you receive will depend upon your physicians request, your response to treatment, and your therapists’ assessment of your need for ongoing treatment.
